With the goal of becoming a trusted resource and one-stop shop for caregivers, we will provide hands-on assistance in finding and accessing services to help the needs of families.
We understand that the disability service system is incredibly complicated and confusing to navigate. We want families to know that we are here to help them gain access to the resources you and your youth with disabilities need to thrive.
Currently, our Resource Navigation program includes:
Transition to Adulthood Office Hours
For caregivers of young adults with disabilities aged 16+
Individualized appointments to help you complete applications necessary to transition to the adult system of care. We work on things such as:
- Guardianship
- DVRS (Department of Vocational Rehabilitation Services)
- SSI (Social Security Income)
- DDD (Department of Developmental Disabilities)
IEP Advocacy
If you want support at school and ensuring your child is getting the support they need through their Individualized Education Program (IEP), we are always happy to attend school meetings with you or talk through the next steps.
